Clinical Practice Guidelines

Doctors HealthCare Plans complies with national coverage determinations (NCD), local coverage determinations (LCD), and general coverage and benefit conditions included in Traditional Medicare laws.  When coverage criteria are not fully established in Medicare statute, regulation, NCD, or LCD; Doctors HealthCare Plans refers to current evidence in widely used treatment guidelines or clinical literature which includes MCG Guidelines, Recognized Compendia as well as other Clinical Practice Resources.

Medicare Coverage Determination Process

Medicare coverage is limited to items and services that are reasonable and necessary for the diagnosis or treatment of an illness or injury (and within the scope of a Medicare benefit category).

National coverage determinations (NCDs) are made through an evidence-based process, with opportunities for public participation. In some cases, CMS’ own research is supplemented by an outside technology assessment and/or consultation with the Medicare Evidence Development & Coverage Advisory Committee (MEDCAC). In the absence of a national coverage policy, an item or service may be covered at the discretion of the Medicare contractors based on a local coverage determination (LCD). Local coverage determinations (LCDS) are defined in Section 1869(f)(2)(B) of the Social Security Act (the Act). This section states: “For purposes of this section, the term ‘local coverage determination’ means a determination by a fiscal intermediary or a carrier under part A or part B, as applicable, respecting whether or not a particular item or service is covered on an intermediary- or carrier-wide basis under such parts, in accordance with section 1862(a)(1)(A).” Refer to the Medicare Coverage Database (MCD) Search at: MCD Search (cms.gov).

MCG Care Guidelines

Recognized Compendia List

The off-label, medically accepted indications of Food and Drug Administration-(FDA) approved drugs and biologicals used are identified under the conditions described below. A regimen is a combination of clinically recognized for the treatment of a specific type of disease process. Off-label, medically accepted indications are supported in either one or more of the compendia or in peer-reviewed medical literature. The contractor may maintain its own subscriptions to the listed compendia or peer-reviewed publications to determine the medically accepted indication of drugs or biologicals used off-label treatment regimen. Compendia documentation or peer-reviewed literature supporting off-label use by the treating physician may also be requested of the physician by the contractor.
